Could possibly be congenital. Foal is nursing normally and owner observed no other problems: Indicates problem is probably not nutritionally related. Mare is current with vaccinations and de-worming: Preventative medicine for the mare may help rule out possible transmission of infectious disease to the foal. Foal has not been vaccinated or dewormed: Neither is required before 6 weeks of age. Foals normally receive passive transfer of immunity from mare which decreases possibility of acquiring neonatal infections. Swollen right hind fetlock: Indicates possible trauma to the fetlock with an inflammatory reaction. Very lame and refuses to use it/toe-touching lame on right rear limb: Indicates fetlock is extremely painful due to inflammatory response and possible damage to soft tissue/boney structure of the joint. Foal resents palpation of the area: Indicates pain stimulus. Foal is even more lame following manipulation of fetlock and does not put foot on ground: Fetlock area again is extremely painful. No instability or crepitance: Can possibly rule out problem with the joint itselfЕ indicates a probable injury to related bones and/or soft tissue (i.e. ligaments) not directly related to the joint. Palpation reveals soft tissue swelling (pitting edema) starting 12cm proximal to the fetlock and extending just distal to it: This indicates injury is in first phase of healingЕvasodilation, increased perfusion to the area delivers inflammatory cells. Also, could possibly indicate tendonitis which causes vascular hemorrhage and pitting edema. Foal does not resent exam with hoof testers: Rules out possible pathology stemming from the hoof. (i.e. injury/abscess) Owner thinks mare stepped on it: Any information from the owner can be relevant to the case. TPR normal?
