This Concept Map, created with IHMC CmapTools, has information related to: CMAP 8, PRE-OP background info Indications (scroll over), INTRA-OP Surgery (scroll over), PRE-OP animal prep Restraint/Position (scroll over), LARYNGOHYOID REDUCTION (tie forward) used for the dorsal displacement of the soft palate (scroll over) Surgical proceedure Complications, PRE-OP (scroll over) General Anaesthesia After the horse has been induced, Position the animal in dorsal recumbency. Clip and surgically prepare the site. with the head and neck extended slightly less than maximally. -The head and neck should be straight helping to keep the incision on the midline. Endotracheal intubation with a 20mm diameter tube will eliminate the need to remove the tube during the surgery. A larger tube may obstruct manipulation of the mucous membrane, and it may need to be removed temporarily., LARYNGOHYOID REDUCTION (tie forward) used for the dorsal displacement of the soft palate (scroll over) post operational Considerations (scroll over), Using: GENERAL ANAESTHESIA If inhalant general anaesthesia is used, after the laryngotomy has been performed, the endotracheal tube can be removed or left in place during the procedure. Especially if it is a smaller tube., using a horse treadmill (scroll over) in combination with Endoscope to observe the soft palate being displaced, LARYNGOHYOID REDUCTION (tie forward) used for the dorsal displacement of the soft palate (scroll over) post operational Care, VENTRICULOCORDECTOMY The Procedure PRE-OP (scroll over), LARYNGOHYOID REDUCTION (tie forward) used for the dorsal displacement of the soft palate (scroll over) pre operational General Anaesthesia, LARYNGOHYOID REDUCTION (tie forward) used for the dorsal displacement of the soft palate (scroll over) medical management conservative management (Scroll over), INTRA-OP Surgical STEPS, Medications For infection ANTIBIOTICS (scroll over), LARYNGOHYOID REDUCTION (tie forward) used for the dorsal displacement of the soft palate (scroll over) pre operational Diagnosis (scroll over), Anaesthesia/ Sedation Using:, Diagnosis (scroll over) can be done by Endoscope to observe the soft palate being displaced, POST- OP ???? Medications, POST-OP Medical Management Medications, LARYNGOHYOID REDUCTION (tie forward) used for the dorsal displacement of the soft palate (scroll over) Surgical proceedure 1) The horse is placed under general anesthesia in dorsal recumbancy and an endotracheal tubes is placed 2) A 15cm ventral midline incision is made extending from the rostral aspect of the basihyoid bone to 1cm caudal to the cricoid cartilage 3) The sternohyoid muscles ae bluntly seperated on the midline and dessection is bluntly extended to the ventral aspect of the larynx 4) The sternothyroid (ST) muscle tendon of insertion on the thyroid cartilage lamina is undermined and isolated 5) A suture is inserted from the tendon and exited fom the lamina cartilage 6) The suture is then placed through the right lamina of the thyroid cartilage and exits more dorsally than the first bite 7) The procedure is repeated on the left side ST tendon was transected 8) Using a suture or wire passer the dorsal sutre is placed ipsilateral and the ventural suture is placed controlateral 9) The horse's nose is lifted so that the head and neck are angled at 90 degrees and the sutures are tied seperatly using a slip knot 10) The head is plaed in is normal resting position and the incision is closed in an acceptable manner
